Reportedly, the gunpowder was placed in a stand with a metal base plate, and it had a hole for a fuse. A piece of wood was placed with colored powder on the top. After that, the tape was wrapped around the metal tubing to create a pipe bomb.
It was planned as such the gunpowder will shoot the powder out of the stand, but the process went wrong, exploding the metal stand instead. The force was so strong that the grandmother who stood about 45 feet away, got hit in the head directly.
